Cannot submit form to email

I'm building a site using FP 2002. Although my site is far from
complete, with only 3 pages done, I've published it to test it as I go. My
hosting site fully supports FP extensions. When I test the form
page...attempting to send it to myself...I receive this form validation
error, "Please correct the information you provided by following these
steps, then submit the information again: A FrontPage File Upload component
is incorrectly configured. Please set the MIME encoding type for the form to
be multipart/form-data."

I tried republishing the page several times and recalculating the
hyperlinks with each attempt, but to no avail. I searched the Microsoft
Knowledge Base for any advice but I came up empty. If someone can point me
in the right direction I would greatly appreciate it. The form page of my

Where is the form page?
If trying to use the FP File Upload it requires the FP 2002 SE and special setup according to
http://support.microsoft.com/default.aspx?scid=kb;en-us;299763&Product=fp2002
