Can't add to the dictionary

I have Entourage 12.2.1 and OSX 10.6.1
I can no longer add words to my Dictionary during spell check.
The add option is grayed out.

What is the fix for this?

The add option is grayed out.

Open the custom dictionary in Word itself, make a slight modification
(add a space and remove it for instance) and save.
It *should* correct the issue.

I did that with no change. I still can not save new words to my custom
dictionary. In fact, even words (like Dunc) that are in the custom
dictionary are shown as misspelled

I even re-installed my Office for Mac and no change there either...

Any other suggestions??

With no change it doesn't work. With no change, WOrd doesn't actually
have to resave the file and doesn't correct the encoding issues.

Sure, the custom dictionary remains and it is still as corrupted as it
was before.
Re-open the custom Dictionary and make a change in it before saving,
