Can't change Icon on shortcut bar

I tried to change the icon on the shortcut bar by right click, properties, change icon. I tried to chose anothe icong or browsw what is availabel. click OK then apply then OK agian and nothing happens
reboot the icon does not change

Why do sometimes the icons more up one or down one? that is, if I am adding a lot of icons in one session,lets say 3 then the icons show an ACT icon but the label is wrong, like each one "slips" down a notch.

Did you click "Refresh Icons" after making the change?

I hit apply, but where is "refresh icons"

Did you click "Refresh Icons" after making the change

Right click an empty area of the toolbar - it is at or near the bottom of
the context menu.
