Can't create a form

I am trying to create a form. I have a query that pulls
from 3 tables and it works fine. When I try to create a
form from that query I get the message "The wizard is
unable to create your form." Help!

Without knowing more about the query and such, it's a bit difficult to
suggest something. Can you provide more details about the query (such as
post its SQL statement)?
