Can't get table headings at top of pages 2 & 3

Had the same difficulty once...discovered that you MUST
include the first line...but I was not including the
first line...a blank one above the one you believe is the
first line...try placing your curser just before the
table and hitting backspace and see if there is an
invisible line or two in there somewhere (above your
table perhaps)...
