Can't mark entries for Index

All of a sudden, I can't mark entries within my docs for the Index. I've
tried running the repair option found under Help. In the past, I've always
gotten the Mark Entry dialog whenever I selected Insert > Reference > Index
and Tables. Anyone else have this problem? Anyone know how to fix it?
