can't print/pdf no-balloon markup - Word 2004 on 10.3.5

I'm trying to create a pdf of a Word file with revisions on but
without the "markup balloons." Every time I try to print and/or create
a pdf ("document show markup" setting), the page is reduced to reserve
room for the balloons (although there are no balloons). The page goes
to normal size when I select "document" without "show markup" from the
Word print settings.

I have balloons turned off under revisions. I've tried using different
printers. I've tried from two different Macs. I've tried PDFMaker via
Acrobat Professional, but it asks where Print Center is (which isn't
on Panther I guess). I'm at a loss. Am I stuck with a reduced image if
I want to print with markup?

Follow-up: After finding this unsolved issue in this and other forums,
and spending the last couple of days on the phone with Microsoft,
we've downgraded to v.X.
