Can't see new tab in custom outlook form when emailed

Hi, Create a custom Outlook form that a user uses to fill in some
information on a student and emails to an individual.

The form has just two tabs: 1- Standard Message Tab 2- Custom Tab
titled: Student Conduct.

As a test I filled in the field on each tab and emailed the message to
myself. When I receive the email and open up the message, the new
(Student Conduct) tab is not in the email.

Any thoughts on what might be going on?
