I wish to add a caption (eg Figure 21) outside (above) a figure ....
but the relevant option is dimmed and I can't explain why !

First, you should always state both your OS and Word versions -
there are lots of permutations, each of which may require a
different answer.
In this case, though, I suspect you've got the cursor positioned in
the paragraph below the picture. Word then knows it can't insert a
caption in that paragraph and have it appear above the picture. Try
inserting a paragraph above the picture and inserting the caption.
You should be able to select the picture and get the option of
inserting above or below. I don't recommend that, as, with at least
some versions of Word, that may enter your caption in a textbox,
which then will not get tabulated in a table of figures (unless you
manually bookmark it).
