CD Template for memorex sticker sheet

I made a design in tif format in PhotoImpact3x. I need however a template
for the cd cover of memorex cd sticker labels. Where can I find such a
thing? I've looked at avery which works well in word. But they don't have
the offset cds (one on top to the left and one on right bottom) Is there a
standard memorex cd sticker template? Thank you!
