Chapter numbers in Master Index

I am creating a document that is made to be revised. It contains
several chapters, and each chapter starts at page 1. The reason for
starting the numbers over for each chapter is because it must be
revisable. If I did it the other way, if I were to add a page I would
have to re-issue manuals to everyone and it would be a disaster.

So here is the question I need a Master Index with chapter numbers. I
know that you can make this happen by using auto number, but I DO NOT
want to use auto number due to all of the problems associated with it.
Is there another way???
