Check Box Form Fields disappear

When a form with 'check box form' fields is viewed in an Outlook 2003 email
all the check boxes disappear.

Due to the wide variety of recipients I need to send the forms in RTF format
in the open email message. This never happened in the WORD '97 version. It
started when we upgraded to the 2003 Office version.
