check box in custom forms are not transfering in emails

I designed a new form in Outlook and I placed checkboxes in the form. But
when someone replies back the check boxes are blank. They are able to
populated the check box but when it is emailed the checked boxes are blank.
Do you have any suggestions?

Did you add the check box to both the compose and read layout, bound to the
same property?
