check boxes on a template

Can anyone tell me how do I add a check box to a template so that users can
check it.? Think of a to do list...

For a To Do list you probably don't want a protected form, so you might want
the type of check box that Word uses on its fax templates. See

There are a number of ways to create and use clickable checkboxes in
documents / templates. Some require protected or locked forms, others do
not. Take a look at the Checkbox template available at
http://www.addbalance.com/word/download.htm#CheckboxAddIn for an exploration
of these.
