check boxes

I'm trying to make a form where I can click on a box and it will fill with a "check" or an "x" (without having to type a character, just with a click). For example, I perform ultrasounds and I want a list of each organ where I can check a box for normal, abnormal, biopsy and then a section for specific written comments. I can't seem to find a form field for this..help!

If you're looking in Insert> Field that explains it The FORMCHECKBOX is
available on the Forms Toolbar along with the Text and Dropdown Form Fields.
