Checkbox Error

Hello,

I am using Word 2002 with Windows XP and I have created a document for my
manager that uses form field checkboxes. The document has worked OK but has
suddenly started giving the following error: "Can't exit design mode because
Control 'CheckBox 1201' can not be created".
This has happened before and I couldn't find the cause so I had to recreate
the document. Now this one has gone the same way. My manager uses Word 2000;
could this be part of the problem? If not, can anyone help?
