You have to use VBA to fill the combo box with entries. You can use my free
VBA Controls Assistant add-in from http://officeone.mvps.org/addins.html to
help you ease this process.

In case you have missed my other message.

I have installed VBA Controls Assistant add-in and then used
Tools/Add-ins but do not see VBA Controls | Insert ComboBox under
Edit.

I must be missing something?

Yes - it is there and is selected.
2. Select "Help" | "About Microsoft Office PowerPoint" menu item, click
"Disabled Items..." button, check if "PPVBA" is listed there.

Nothing listed as disabled.

Where next?!
