Code sample for State/Country dropdown?

Is there any code for creating a dropdown that is already populated
with US states &/or Canadian provinces? Also, one with countries.
This would be similar in function to the type of dropdown you would see
on any online shopping form.

One way to achieve this is to keep the list in a DB and to populate your
drop-down through a Web Service.
