Combo Box errors - duplicating value in Primary Key -- can't save

I have set up several Combo Boxes that reference indexed (no dup) Tables.
Whenever I make a selection from a bound Combo box list and try to save, I
get an error message that says I am trying to create a duplicate value in the
Index or Primary key.

When I look at the Combo Box in the Datasheet View, when you click on each
row of the list, each row field has it's own pull down list that shows all
rows in the table.

Have I set up the Combo Boxes incorrectly?

Probably not. First look in the table. Are there combo boxes there? There
shouldn't be. They are there because of using Lookup Fields:

If that's not the case, you may have bound the combobox to a primary key
instead of a foreign key.
