Command to generate Form fails

Generating a Report via a command button in a form causes the following:

<Message_Box>:
Enter Parameter Value
[<form_name>], 1[UnKnown]=[Injuries
<area_to_enter_value>
<OK_button> <Cancel_button>

this is followed by:

<Message_Box>:
Microsoft Access
Too few parameters, Expected 0.
<OK_button>

Any idea where to start looking for the problem?
