??? Confused on Contacts Form

Hello,
I am using Outlook 2000 on XP Home. I want my Outlook Contacts
form to be different. In particular, I want to have three email
addresses showing, instead of using the drop-down fields. I also want
to change the order and names of two of the phone number fields.

I opened up the forms tool and designed the form to my liking.
Then I saved it. Now I can't find it.

I'm sure I'm missing a basic step -- how do I find my form and
make it the default?

Or how do I modify the default Contacts form and save it so that
I get it all the time?

If you made changes and saved, you probably saved an item, not the form
separately. Maybe you have an item without a subject or one that's larger
than average? That could be the one containing the form definition. Once you
find and open it, choose Tools | Forms | Publish Form to publish it. See

(Not even sure what an "item" is).

An item in this case is a contact stored in your Contacts folder.
