I am looking for a template that features (for starters) slanted boxes,
aligned
horizontally on the page (standard format used by the big firms), in which I
can list the 5 to 7 phases of the project.

Before I can reply, I need to ask a few questions.

1. Which "big firms"?

2. Are these boxes on a Gantt Chart or a Network Diagram or a WBS or
something else entirely?

I am not sure what you would call it but I once saw it used by CapGemini (I
did not work for them). I don't think it would be a gant chart as that has
too much detail. The initial box chart only consists of 7 or 8 boxes on a
page lined up horizontally and slanted to the right. Does that help?

Not much. You could just use some kind of graphics software to do that.
Or a Project Network Diagram with the auto layout turned off. Do you
know what kind of functionality it had?
