I have created a custom Contact form. When I send it to myself, I do not get
the same format as the original. It comes up with some previous form that I
had created but deleted. Every time I try to change the saved form to look
like the original, it reverts back to the previous old form.
Where is Outlook getting this form from? I have gone through all folders and
can't find it anywhere.

I then tried to create the custom form as a Message form. When it is sent,
it just shows subject line on the screen and the body of the message. When I
select to print it, it shows all the fields that have been filled out. How
do I get this to display on screen as we are trying to create a paperless
form?

How do I compose the read layouts and make it the default for that
particular form?

What steps are you following to "send it to myself"?

Message forms by default have separate compose and read layouts. It sounds
like you customized only the compose layout.

In design mode on your message form, click the Edit Read Page button.

There is no "default" layout. Either you have one layout that always
appears, or you have two separate layouts for new and existing items.

If you're talking about the original contact form, what would you want a
Send button to do? If about the message form, same question -- what would
you want a Send button on the form to do that the Send button on the toolbar
doesn't do?

Sorry I should have clarified it. The form is a Contact form. I would like
to be able to send the Contact form to a distribution group once it has been
filled out by the User directly from the form rather than saving the contact
and then attaching it to an email.

It's OK, I got it to work. I created a toolbar with the Forward button set
on it.
