Contacts lose category & repeated "microsoft error reporting" window

I am currently running Microsoft Office 2008 on a 2.4 Ghz Intel
Macbook running 10.6.1

I upgraded to 2008 several weeks ago after dealing with the Time Zone
sync bug between Entourage 2004 & iCal in the new Snow Leopard.

I upgraded fine, the Time Zone bug was instantly resolved and I've had
no problems syncing between Entourage 2008 & iCal/Address Book. These
in turn sync to my iPhone & MobileMe.

I noticed this afternoon that at some point over the last 5 days or
so, Address Book & my iPhone had deleted all Address Book groups
except for 1. This in turn deleted all Entourage contacts except
those that had existed in that Group.

I went to work fixing this by pulling the missing groups back into
Address Book via Time Machine.

Of course, this required that I re-assign the categories within
Entourage after pulling those back in.

While doing most of this, I still had "sync with Address Book" checked
within Entourage so that as I fixed AB, it would sync those changes
back to Entourage. I had already turned off MobileMe syncing for
contacts until I had a chance to get everything fixed. At one point,
I had some things that I wanted to try to fix WITHOUT this syncing
between Address Book & Entourage, so I turned it off in Preferences.

Now I want that back on again, but I get a continuous loop of the
"Microsoft Error Reporting" window. As soon as I have it "send" the
error information, it pops back up with a new window. At one point
earlier today, those were popping up so fast that I had about a dozen
on my screen at once.

In addition to getting the "Microsoft Error Reporting" window, it has
reset all of my Entourage contact categories back to "none". The sync
didn't DUPLICATE the contacts--I don't have one with the correct
category and one with NONE--I simply have the one with no category
assigned.

How do I get out of this vicious loop? I had to go into Entourage
preferences and turn off contact syncing again in order to escape the
constant popping up of the "Microsoft Error Reporting" window. And I
hate to go through all the work of changing the Entourage contact
categories only to have them reverted back to "none" immediately.

Any ideas on what I need to do? Until today I've never seen this
message--now it won't stop appearing.

A little more information. Found another similar thread. I turned
off "sync contacts" in Entourage preferences, then turned it back on
and let it overwrite Address Book.

The "Microsoft Error Reporting" window continues to come up. I
temporarily ignore the message. I enter a new contact in Entourage--
immediately an identical message appears, but Entourage successfully
syncs the new contact over to Address Book. Same happens in reverse
when a new contact is added to Address Book

So syncing seems to be working okay, but something seems to be
erroneously triggering the "Microsoft Error Reporting" window--it
happens every time I add a new address to Entourage OR Address Book,
but it still successfully syncs. This means that Microsoft
SyncServices is constantly getting rebooted every time we dismiss the
"MER" ("Microsoft Error Reporting") window.

"entourage.syncservices12"--during the time period that I was trying
to correct the sync issues:

Here are the recurring Console messages from the few days prior to
noticing the problem--dates show today, but the entry contents show up
in previous days also. Actually these entries seem to appear back in
time to the point where the original AB groups were deleted. I
retrieved the missing groups from a Time Machine backup from
10/26/09-8:19 pm. The below entries in the Console began appearing
within the hour after that last good/accurate TM backup.:

Try deleting this file. It's obsolete in Snow Leopard.

Do I delete the entire "24U Appearance OSAC.osax" plugin or just the
individual MacOS component inside the contents folder?
