Contacts spreadsheet

I have a spreadsheet in CSV format suitable for importing into Outlook
Express. I am trying to import this into AD using CSVDE but I notice that
the format of the file is not correct. Can anyone point me in the general
direction of where to get a template for importing into a certain OU in the
AD and therefore into my GAL?
