Convert document to a fillable-form

I am using Office 2003 on Windows XP.

I have a MS-Word document that is essentially a form. What I want to do is
add text boxes, check boxes, and option buttons directly in the document so
people can fill it in (not on the web).

When I try to add a text box, for example, the text near the box jumps out
of place. How can I keep it from doing this? Also, the text box width is
small, how do I make it larger?

Also, can someone please post a link or links to web sites that help you
with this sort of thing?
