convert existing contacts to customised form - Outlook 2003

Hi,
I've been able to create a customised contact form and it is now the default
form when I double click to create a new contact. Awesome....
But now, how do I (in Outlook 2003)
1: convert my exisitng contacts to the new customised form
2: if I were to import contacts from excel, how do I get these defaulting to
my customised form?

Not from my computer, it isn't. The site comes up immediately. Both
