Convert Word to PDF with Hyperlinks for INDEX Entries?

How do I configure PDFMaker or Distiller to make
hyperlinks for each entry inthe <INDEX> in my
Word document? These tools can make hyperlinks
for my Table of Contents entries, but how do I
get them for an index, or for multiple indexes?
