Converting *.doc to a "fill in the blank" form I can send via Outl

I would like to create a "fill in the blank" and send it via email. It seems
that I can do that using forms in Outlook, but I am not getting the design
process. Does anyone have a template? At this point I am not concern about
how the data is collected (just that it is) so I would need a submit button
to have the reply come back to me.

Outlook is not a good survey solution except in the very narrow scenario where Exchange is the mail server, all recipients use Outlook, and you can publish the form to the Organizational Forms library. If you're using a version of Office before Office 2007, sticking with a Word document might be a better choice. (A web page survey would be even more broadly usable.) A good option is also available if you use Access 2007 + Outlook 2007.
