Could not Insert UserForm into VBA

Hi,all

I met a trouble on inserting a UserForm into VBA (Excel & Word).
While I tried to do so in Excel or Word, the application exits with an error
report. Modules or Module classes are OK.
And I noticed that the EXCEl with version 10.2614.2625(VBA version 9108
Retail 6.3.8863) works correctly!
My Excel version is : 10.5815.2625 and VBA is 9969 Retail 6.4.8869.

How to solve this ? Thanks a lot.
