Coulmn Headers

Hello,

I have a set of shipped data based on ship date; I need to represent the
data in following format: Columns headers will have the text “Shipped†then
the calculated work week from the ship date followed by the year.

Example:
Shipped
Ww: 45-2006

I need it in form or query output but not in report?
