This is Word 2007, correct? (You should always say what version you're
using.)
Both the cover pages and the things in the page number gallery are "building
blocks", and the default set are supposed to come from a file named Building
Blocks.dotx. You may have a damaged or incorrectly installed copy of that
file.
First try going to Insert > Quick Parts > Building Blocks Organizer. Can you
see any entries in that dialog? There should be several dozen, including
ones whose Gallery entries say "Cover Pages" and "Page Numbers".
If there are no entries, or just a few, try going to Office button > Word
Options > Resources and clicking the Diagnose button. That runs a
diagnostic/repair routine.
If the entries are still missing, shut down Word. On the Start menu, click
the Computer button. In the address bar at the top, type or paste in
%appdata%\Microsoft\Document Building Blocks
and press Enter. You should see at least one folder named with a number that
represents the default language of your Office installation, such as 1033
for US English. Inside that folder, find and delete the Building Blocks.dotx
file. Then restart Word and again open the Building Blocks Organizer, which
should cause Word to make a fresh copy of the original file. Does that help?
