Create a Fill-in Form , which can be returned completed - Outlook 2003

I have been attempting to create a Fill-in Form that the sender can
complete some information, send the form and have the recipient
complete the form and send it back- not as an attachment. We are
restricted from using attachments.

Every attempt in creating a simple fill-in form (maybe 10
questions ), has resulted in a black respose coming back to the
sender- or an error when attempting to return a Published form.

Is there a way to create a fill-in form and have the respose return
the completed form??
