create a form...multiple pages

How can I create a form that automatically keeps identifying information such
as a name/address etc on top of pages 2 and on. (i.e once this info is
entered once on page 1... it needs to be automatically added to page 2 and
on). .....sort of lika a "header" with the ability to accept form
fields.----maybe stupid question but i am a novice, sorry

In Excel -- put that information in a row... and then from the Page Setup
Dialog Box ... Sheet Tab... enter the rows you want to repeat at the top of
each page.

In Word -- you'd have to put it in the Header...

If you are using form fields in your document, in the Properties of the form
field modify the Bookmark for each of the fields you want to use in the
header and give it a more meaningful name. In addition, select the
"Calculate on Exit" option. Then in the Header, add cross-references to the
bookmarks. After you setup your header create a First Page Header (in Page
Setup) so your first page is blank and pages 2 - ? will contain the
information in the form fields.
