Create a form that user completes in Word & print 3 identical Xs

I would like to create a form, occupying ~ 1/3 of a page, have the user type
in the requested info and then have that form repeated two more times on the
same page, including the initial prompts (questions) and the user input.

If this is possible, can someone tell me how? Should I create a Word doc to
be completed or a template? If I do a template, would I make a 3-up
identical copies and can the info input be duplicated on the other two? If
this question is in the wrong area, please somebody tell me!!

I have had a terrible time trying to find my way around this site. Am I
