Hi, Troy,
Use a Word table for your form. Put the field headings
(i.e., "Name:" in one cell and leave the space to be
filled in as a separate cell (you can use cell borders
for the places you want to underscore the form response
and they will remain in tact). Be sure to bottom align
the cells -- so the response will appear to sit directly
on the line (border) -- and the field heading will be on
the same horizontal line as the response.
