Create a form with a merge prompt

I know how to create a template message, by creating an .oft file. However, I
wanted to know if it's possible to insert a merge prompt into a template,
which allows me to prompt for data prior to the message being sent. I.E. An
order number changes, and everything else is the same on the template.
