Create a survey?

Is it possible to create a 25-30 question survey using the form controls in
publisher 2003?

You should be able to do this provided your web host has FrontPage Server
Extensions (FPSE), and you upload using HTTP protocol. For more information
read Publisher web publication forms 101 , the third article down:
