create Adobe PDF toolbar

I have MS Word 2000 v9.0.6926 SP-3 all the patches and Adobe Acrobat
v4.0 0318 1659. On the Toolbar, PDFMaker 4.0 isn't creating Adobe PDF.
This worked in the past. In Word, menu, "File - Creat Adobe PDF ..."
also doesn't work. It is listed under File twice and neither works. In
Word I ran and completed "Detect and Repair"; the problem still
exists. Seeking assistance. Thank you all in advance.

The toolbar/menu items are created by an add-in that
typically is part of Adobe Acrobat installation. In Word,
try going to Tools > Templates and Add-Ins and make sure
the pdfMaker box is checked (if it is listed at all). If
it is not listed, or if it is listed and checked but you
still do not see the icons/menu items, you could uninstall
and reinstall Acrobat, being sure during reinstall that
you are not excluding any options to include a toolbar
icon/menu item.

could it possibly have something to do with it listed twice in the
file menu?
it worked in the past and to make PDF file it must print to distiller,
that is still in with all printers.
I do see the icon; would have to search for adobe program to
re-install. do you or anyone else have idea how to remove item from
file menu since it there twice?
