Create an Advanced Form

Hi there,

I'm looking to set up a form where users can input a Job Number and select permissions.

For the permission section, it should offer a yes or no option. If the user selects "no," I'd like three additional columns to appear. Following this, there will be a designated amount for each user. Essentially, there will be three additional columns: Cost for Labor and Cost for Cups. Assuming each user is allocated 50 units, if a user inputs 25 for labor and 26 for cups, it should be accepted. Can you assist me with this?
