Create Document

The president of our organization has a report we must submit every month.
This is a word document and the questions are the same every month. We can
only type in the sections to update our information. Her questions are
"locked" and you cannot edit any information i.e.: How many attended? 7.
I can type as many characters as needed in my answer section, but I cannot
"edit" her question section.

I would like to learn how to create this kind of document. Can you help me
please? I am using Office Word 2003 SP 3.
