Create Interactive survey with Office

I’m looking to create an Interactive survey that will use radio buttons,
drop-down boxes, and textboxes to answer some 50 or more questions. Staff
will use the survey to gather information from customer, concerning their
experiences with the company.

I am looking to create a survey that will be accessed/saved on a local
network…not the Internet…. I would like the staff to be able to easily access
the survey from a template, if possible, and saved to either an Access/Excel
database.

Your suggestions would be truly helpful. Also, I have researched FrontPage,
Access, and Excel, but I cannot see any easy way to create a form for a local
network, using these programs.
