Create New Task gets added TWICE

My resource has added a new task (vacation time in our "administrative plan"
as suggested by msProjectExperts).

After saving the new task, the task appeared in the resource's "view my
tasks" window. He highlighted it and clicked "Update Selected Rows." I
received an email alerting me to approve the changes, which I did. I then
saved the "administrative plan" project and republished it.

After republishing the project, I could see the new vacation task in the
project plan with the proper schedule and hours. However, in my resource's
"view my tasks" window, the task shows up TWICE!! This is all part of a
proof of concept demo so we know that there is no other vacation task
anywhere in the "administrative plan."

What could be causing this? Can it be fixed?
