createing forms

Which program should I use to create an office form for my wife that she can
use to record patient information on? And what or how do you get this form to
work? what I am trying to say is she records all kinds of medical data during
a patient assessment i.e. weight, height, age ect. how can you make this form
with the base information on it, and she just has to tab through the sheet to
the box she wants to record data in? I made the exact form in Power point but
it was to difficult to use with all the text boxes, one bad move and you drag
the box out of position,HELP!!!!

hi
i would concider access. it has a forms wizard that will create your form
for you. just set up your table first. you don't have to put data in the
table first but you do need headers. and there is a report wizard that will
create a report to view the input data.
