Creating a Custom Bullet

I would like to create a custom bullet. to look like the one below, i also
want the option to add information in the 3 different spaces.
is this possible to do in Microsoft word 2007?

Not in the way you describe, no. Automatic "bullets" cannot be edited, so
you can't create "fill-in-the-blank" lines as part of a numbering scheme.

You could insert the lines as an AutoText entry, but you still wouldn't be
able to type *on* them.

You could create a six cell table with bottom borders on cells 1,3 and 5
cells 2 and 4 made narrow enough to take the / character. Set a fixed height
appropriate to the text size and make the cells an appropriate fixed width.
You can then save that as an autotext or autocorrect entry and you would be
able to type on the lines.

now if i save this as an auto text when i hit Return will it number my lines

A SEQuence field is means of auto-numbering. Each time you insert the field
it will display the next number in sequence thus you could use
GDS {SEQ GDS}.

which inserted into the document would give

_____/______ /______ GDS { SEQ GDS }.
_____/______ /______ GDS { SEQ GDS }.
_____/______ /______ GDS { SEQ GDS }.
_____/______ /______ GDS { SEQ GDS }.
_____/______ /______ GDS { SEQ GDS }.
_____/______ /______ GDS { SEQ GDS }.

which would display as

_____/______ /______ GDS 1.
_____/______ /______ GDS 2.
_____/______ /______ GDS 3.
_____/______ /______ GDS 4.
_____/______ /______ GDS 5.
_____/______ /______ GDS 6.

The only proviso is that if you save the SEQ field in your
autotext/autocorrect entry, you would have to update the fields in order to
display the correct result of you would see

This isnt working for me. i create switching orders like the one below
everyday, and i have to print them off to write in the initials and
time....any other suggestions on how to make this work???

That's what comes of supplying only half the required information.

In order for this to work you are going to need a much larger extra last
column for the table to take the instruction text (on one line).

The only alternative I can think of will not work if it is necessary to
*type* on the underlined sections. Then you could use a macro to enter the
underlines and the number field etc
