creating a form in word 2000

Hi all,

I have a form that I want to send to my clients and want to put checkboxes
in some of the questions.

For example:
( ) Winter ( ) Spring ( ) summer ( ) fall
I would like to have them check the different selections that apply to them.
Is this possible? If so, How can it be done.

Any help would be much appreciated. Thanks,
