Creating a form template

I am trying to create a form template in Word 2003. Everytime I open the
template, there are extra, blank fill in boxes that I did not put in there.
How do I keep this from happening (I think it has something to do with the
header) or at least, how do I show the code for the boxes so I can delete the
extras or alter the ones I want left in there like I could in older versions
of Word
