creating a form

the first link was helpful except that the form i'm trying
to make needs blanks in the middle of sentences. so when i
use form feild, it makes all the lines in the paragraph
shift. is there any other way to do this so that i can
fill in blanks in mid-sentence?

If you're inserting text in the middle of a sentence I would think that you
would want the sentence and paragraph to reflow and rewrap naturally just as
it would if you were adding text to an ordinary paragraph.
