Creating a Form

I have a preprinted paper form I load into my printer. Is there anyway that
I could take a scanned copy of my preprinted form and import it into
Microsoft Word? Then make a form by placing the scanned copy in the
document and then placing the fields in the proper location. Then once I am
ready to print it will only print the text and not the form that is in the
document and my hardcopy would have the text in the proper location.

I am using Microsoft Word from the Office 2003 Professional Edition
