Creating a lending contract

Hi I need help today its urgent I must draw up a contract for one of my students who is getting a lend of a laptop computer. She will be solely responsible while the computer is in her care. I need her to sign and understand the responsibility she wil have. I just need to word it right so if anyone out there can help I would appreciate it
